Job Title: RN/Registered Nurse - Clinical Care Coordinator Facility Description: This employer is a skilled nursing facility located in extreme Western MN, near the SD border. There is also an attached clinic and hospital. City Description: This is a great small rural community, population 1,700. It is the county seat. Excellent K-12 school system. Low cost of living! Commutable to other employers within a 40 mile radius. Position Description: This is a RN position at the leadership level. Titled "Clinical Care Coordinator", the primary focus is to direct resident care and contact families with any health concerns of their loved ones. In addition, they are the primary contact with the physician and report on progress or change of status for residents under their care. Average daily census of 35 patients.
